COVID-19 Preparedness Response Plan

COVID 19 PREPAREDNESS AND RESPONSE PLAN

  1. All patients are phone screened prior to making an appointment per prepared protocol
  2. Only patients are to enter waiting room unless unique circumstances require an attendant
  3. Patients entering waiting room must have a mask or the office will provide one
  4. Waiting room population is to be kept to a minimum and patients can be instructed to wait in car as needed to be called
  5. Written patient screening questioner (and guest if accompanied) to be completed by patient and attendant and signed before entering the office on each and every visit, with appropriate action to be taken if questioner is failed by either patient or attendant
  6. Temperature check on all patients (and guest if present) before coming back into treatment area. Any temperature 100.4 or higher will be rescheduled
  7. Hands and or gloves will be sanitized at entry with hand sanitizer
  8. Patient and attendant if present will get a sticker to be worn during appointment indicating they have been appropriately vetted
  9. Patients shall move though the office with the best effort to keep social distancing before, during and upon exit from their appointment
  10. Exam rooms, waiting rooms counter tops common areas shall be thoroughly disinfected regularly throughout the day and exam rooms after each patient use
